Circle Home Care was created to provide a continuum of care, a complete circle of care to meet the growing and changing needs of this community. Circle Home Care services Wythe, Bland and surrounding counties.

As a service of Wythe County Community Hospital, Circle Home Care works with the hospital, physicians and family members to ensure the needs of each individual are addressed. This more personal approach to health care enables many individuals to remain in the comforts of their own home while receiving the same quality of care they would at Wythe County Community Hospital.

More and more medical services are being made available in the home. Choosing home care can prevent an unnecessary hospitalization or allow you to leave the hospital sooner. Home care also may allow you to live at your own home longer.

Services Offered

Circle Home Care offers a variety of health care services to meet the individual needs of its clients. All services are given by licensed or certified professionals who have a special interest in caring for people in a home environment.

Skilled Nursing Care

A registered nurse or licensed practical nurse provides treatments and procedures requiring specialized nursing skills. This may include conducting a skilled nursing assessment of needs and problems, administering intravenous medications, and keeping your physician informed of your condition. The nurse also plays an important role in educating you about your condition and teaching you and/or your family members skills to care for you.

Home Health Aide Care

A certified nursing aide assists you with personal care under the direction of a registered nurse. This caring person may help with bathing, personal hygiene and meals. He or she may also work with you in carrying out an exercise program designed by a therapist.

Specialized Therapy

A registered physical therapist, occupational therapist or a licensed speech/language pathologist can offer you specialized care to help you regain or maintain certain functions. Whether helping you with mobility, function or speech, these therapists tailor a program to meet your specific needs.

Medical Social Services

A social worker provides support as you and your family members adjust to changes in your life. This professional also can connect you with resources within the community.

Caregiver Support Group

Circle Home Care is the founder of the Caregiver Support Group, providing emotional support to those caring for elderly, sick or disabled family members or friends.

Cost

Because home care is a convenient, cost-saving alternative to hospital admissions and extended hospital stays, it is covered by Medicaid, Medicare and most private insurance plans.

Medicare and Medicaid will cover home care services if your physician orders the services and you are unable to leave home without assistance.

Private insurance companies do vary in the coverage they provide. Circle Home Care will work with you to review your policy and determine what services will be covered. For your convenience, Circle Home Care will bill the insurance company directly.
